2009 ASA/USA Softball Individual Registration Form

This form is used to register your team with the ASA and for providing the individual insurance. Directions are located by clicking on the ASA Registration Tab at the left.

2009 ASA Championship Roster

This roster can be printed and used as the Official Roster and Waiver of Liability. The 2 pages of this form MUST be printed on front and back of the same sheet of paper. When obtaining signature from your local JO District Commissioner, please have original and 3 additional copies available. 2009 ASA Pick-up Player Form

Teams are permitted to pick-up three (3) players as follows:

Pick-up players must come from an ASA registered team who is no longer eligible for Championship play.
Pick-up players must be from the team's same division (Girl's Fast Pitch) and the same or lower classification (this includes the age of the team as well as the Class of the team.)

When obtaining signature from your local JO District Commissioner, please have original and 3 additional copies available. For any questions regarding this roster form, please contact Renee Williams.

Bollinger Individual Insurance Claim Form

This form MUST be filled out, sent to NE ASA, 4103 Osborne Drive East, Hastings, NE 68901. Form will be sent back to you for you to put with your other forms and then send all items in together to Bollinger. Forms sent into Bollinger without a signature will be significantly delayed.
