ASA Registration Information for LYSA Teams:


ALL teams playing in the LYSA league MUST register individually.

 Form MUST be filled out via your computer.

 Download this 2009 ASA/USA Softball Individual Registration Form to your computer.  Forms are due to Renee by February 28, 2009.  This form is NOT your ASA roster.  Click on ASA Docs & Forms on the left for your ASA Roster.
Mandatory for one (1) adult coach/manager to register with each team.  Please remember you MUST list the coach/manager in the body of the form even if it is the same individual listed at the top of the page.

You may register as many coaches as you wish as long as the fee is paid for each and they are listed in the body of the form.

Fee is $13.00 per player/coach.  Fee includes registration to ASA and a $0 deductible secondary insurance policy.  To learn more about insurance, please click on the ASA Insurance link on the left.

Save form to your computer as your team name-age-class (i.e. Huskers-14B).

Email form as an attachment in excel back to Renee at sb.blue@yahoo.com.

I will email back a verification after receiving your email.

Make checks payable to Renee Williams and send to me (at the address listed on the form) with a copy of your 2009 ASA/USA Softball Individual Registration Form.  Please include your team name and team age on the memo line so I can credit the proper team.  Click on the Contact Info Tab for my contact information.

I will also email back a verification after receiving your form and check in the mail.

Your team is NOT registered until I receive ALL items above.  The email attachment of your 2009 ASA/USA Softball Individual Registration Form, a check in the proper amount and the hard copy of your 2009 ASA/USA Softball Individual Registration Form.

Your team will be provided with one (1) 2009 ASA Rule Book, Balls and Strikes Magazine, Score Book and the 2009 Nebraska ASA Code & Procedural Book.

 To add additional players/coaches at a later time, download a new form, fill out the top, add the new players/coaches and follow the above directions.
